We sought to measure the proportion of publicly insured children who receive outpatient care after their discharge from the emergency department, determine factors that predict this outpatient follow-up, and evaluate the relationship between outpatient follow-up and subsequent use of hospital-based healthcare services.
During 2019, a cross-sectional study involving pediatric encounters (<18 years) was conducted based on the IBM Watson Medicaid MarketScan claims database within seven U.S. states. A follow-up visit at our ambulatory clinic was prioritized within a timeframe of seven days following the patient's emergency department discharge. Re-admissions to the emergency department and hospitalizations within a seven-day span served as secondary outcome variables. Multivariable modeling techniques included logistic regression and Cox proportional hazards.
We incorporated 1,408,406 index ED encounters, with a median age of 5 years (interquartile range 2-10 years), and a 7-day ambulatory visit occurred in 280,602 (19.9%). Conditions requiring 7-day ambulatory follow-up at the highest frequency included seizures (364% of cases), along with allergic, immunologic, and rheumatologic diseases (246%), other gastrointestinal diseases (245%), and fever (241%). Younger age, Hispanic ethnicity, discharge from the emergency department on a weekend, prior outpatient visits before the emergency department visit, and diagnostic tests during the emergency department visit were all factors linked to ambulatory follow-up. The presence of ambulatory care-sensitive or complex chronic conditions, along with Black race, was inversely related to ambulatory follow-up. Analysis using Cox models demonstrated that patients with ambulatory follow-up had a heightened hazard ratio (HR) for future visits to the emergency department (ED), hospitalizations, and return visits to the ED (HR range: 1.32-1.65 for ED returns, 3.10-4.03 for hospitalizations).
Among children discharged from the emergency department, one-fifth subsequently had an ambulatory appointment within a week, a rate that varied considerably based on individual patient traits and diagnoses. Children receiving ambulatory follow-up care experience an increase in subsequent healthcare consumption, including emergency department visits and hospitalizations. These results underscore the requirement for additional study on the function and costs of routine post-ED visit follow-up appointments.

A clinical investigation was undertaken to determine the outcome of using subcutaneous semaglutide, a glucagon-like peptide-1 receptor agonist (GLP-1RA), once per week, for up to two years on individuals with type 2 diabetes (T2D) in standard clinical settings.

From the total population, 9284 individuals redeemed at least one semaglutide prescription (intention-to-treat); meanwhile, a further 4132 individuals obtained semaglutide prescriptions continuously (on-treatment). The on-treatment cohort's characteristics included a median age (interquartile range) of 620 (160) years, a median diabetes duration of 108 (87) years, and a baseline HbA1c level of 620 (180) mmol/mol. From the group receiving treatment, 2676 patients underwent HbA1c measurements at the beginning of their treatment and at least one additional time during the subsequent 720 days. The mean change in HbA1c after 720 days was -126 mmol/mol (95% CI -136 to -116, P<0.0001) for patients without prior GLP-1 receptor agonist (GLP-1RA) use, and -56 mmol/mol (95% CI -62 to -50, P<0.0001) for those with prior exposure.
